The personal data I collect about you will include data relating to your name, address, date of birth, medical provider (GP) and data relating to your health. I will process your personal data in a manner that will allow me to provide you with a personalised hypnotherapy service and assess and devise a programme of therapy with the intention of resolving the personal issues which you have disclosed in communication with me.
